Q. How can I become a Participating Provider with IHG?
A. You can request an application by either contacting
our Customer Service Department at 866-511-4757 or requesting
an application on our website.

Q. Can I get a listing of Client/Payors in my area?
A. Please contact your Provider Representative located
in your region.

Q. Where do I send claims?
A. Mail claims to the address listed on the Member ID
Card.

Q. How do I appeal the way a claim was paid?
A. IHG recommends contacting the payor to verify the claim
was paid in accordance with plan benefits. If you feel
that the claim was processed in error, please contact Customer
Service Department at 866-511-4757 for instructions on
how to appeal the claim.

Q. How do I identify an IHG member?
A. IHG
makes every effort to identify participating PPO members.
Most members have a Member ID card that contains IHG’s name
and/or logo. Click here to
see a sample ID card with the IHG logo.

Q. How do I verify benefits?
A. Please contact the payor listed on the Member ID card
for benefit information.

Q. My state has a standard application form; will you

A. No. IHG verifies providers by their FEIN#.
